Responsibilities

• Conduct physical therapy evaluations and treatments via telehealth
• Develop, implement, and monitor individualized care plans
• Work closely with onsite therapy staff and the interdisciplinary team to ensure optimal outcomes
• Document therapy sessions and progress in compliance with regulatory standards
• Provide education and support to residents and caregivers through virtual sessions

Qualifications

• Active California Physical Therapist (PT) license (required)
• Experience in skilled nursing, rehabilitation, or post-acute care preferred
• Comfortable using telehealth technology and electronic documentation
• Strong communication and problem-solving skills
• Compassionate, patient-focused, and team-oriented
