MDS Nurse (Healthcare)



Peninsula Post Acute is looking for a dedicated and detail-oriented MDS Nurse (LVN) to join their team.

About the role:

This crucial role focuses on managing the Minimum Data Set (MDS) process, which is essential for resident care planning and facility compliance with federal and state regulations. You'll play a vital role in ensuring residents receive the highest quality of care and that accurate documentation supports the facility's operations.

Key responsibilities:

  • Collecting and completing MDS documents within required timeframes.
  • Reviewing and comparing entered data against original documents to ensure accuracy and making necessary corrections.
  • Maintaining organized and up-to-date resident charts by filing MDS documents appropriately.
  • Protecting sensitive patient care information and maintaining strict confidentiality at all times.
  • Collaborating with the interdisciplinary team to develop, implement, and evaluate personalized care plans.
  • Ensuring compliance with federal and state regulations, including PDPM guidelines.

What you bring:

  • Current and active Licensed Vocational Nurse (LVN) license.
  • Strong understanding of MDS regulations and processes.
  • Excellent assessment, organizational, and critical thinking skills.
  • Proficiency in maintaining accurate medical records and documentation.
  • A commitment to providing compassionate patient care.
  • Ability to work effectively under strict deadlines.
