We are seeking a strong, hands-on leader to join our growing SNP Care Management team as Supervisor, Care Coordination. In this fully remote role, you'll oversee a team of non-clinical Care Coordinators and ensure timely, high-quality care management for members with complex, chronic needs enrolled in our Special Needs Plan (SNP).

You'll play a key role in executing our CMS-compliant Model of Care - with a strong focus on performance, team development, and member experience.

We are seeking a strong, hands-on leader to join our growing SNP Care Management team as Supervisor, Care Coordination. In this fully remote role, you'll oversee a team of non-clinical Care Coordinators and ensure timely, high-quality care management for members with complex, chronic needs enrolled in our Special Needs Plan (SNP).

You'll play a key role in executing our CMS-compliant Model of Care - with a strong focus on performance, team development, and member experience.

Key Responsibilities

  • Supervise, mentor, and support a team of Care Coordinators
  • Ensure timely completion of Health Risk Assessments, Individualized Care Plans, and Interdisciplinary Care Team meetings
  • Monitor workflows, assign caseloads, and manage priorities based on reporting
  • Identify gaps and lead process improvements in coordination activities
  • Prepare for and participate in internal/external audits (CMS, vendors)
  • Collaborate with leadership to drive performance and compliance
  • Promote engagement, accountability, and growth across the team
  • Time card management and metric reviews

What You Bring

  • 3+ years experience in care coordination or healthcare support
  • 1+ year in a supervisory or team lead role (healthcare setting preferred)
  • Strong knowledge of managed care, medical terminology, and CMS guidelines
  • Ability to analyze data, manage multiple priorities, and drive team results
  • High school diploma or GED required; additional healthcare education preferred
  • Proficient in Microsoft Office

Pay Range: $58,531.00 - $87,797.00
Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
