Care Manager (Position Located in Modesto or Merced) (Manufacturing)
Assess client needs across health, behavioral health, trauma history, social supports, housing, and employment.
Develop and oversee Individual Care Plans (ICP) and Health Action Plans (HAP).
Provide services in-office, via telehealth, or through field visits based on client comfort.
Coordinate care with providers, social services, and community supports.
Apply evidence-based practices such as Motivational Interviewing, Trauma-Informed Care, and Harm Reduction.
Collect and interpret behavioral health and psychosocial data.
Facilitate community linkages and adjust referral processes to fit cultural and community needs.
Empower clients through advocacy and coaching.
Document all services in a timely and compliant manner using Electronic Health Records (EHR).
Participate in program meetings, trainings, and agency initiatives.
Maintain HIPAA compliance and adhere to all agency policies.
Qualifications
Prior experience in care coordination or case management required.
High school diploma or equivalent required.
Knowledge of local community resources and social services.
Strong interpersonal, active listening, and communication skills.
Ability to exercise sound judgment and decision-making.
Team-oriented and collaborative mindset.
Proficiency in Microsoft Office, Office 365, Teams, Zoom, and EHR platforms.
Valid driver's license and access to insured, reliable transportation.
