Thats why our Integrated Behavioral Health (IBH) team is embedded directly into our medical clinics, working side by side with physicians, nurses, and care coordinators to treat the whole person, not just the diagnosis.

Were looking for a Licensed Clinical Social Worker (LCSW) who thrives in a collaborative, multi- specialty environment and is passionate about breaking down barriers to care. In this role, you wont be stuck behind closed doorsyoull be in the room where it happens, making real-time, life-changing interventions for patients when they need them most.

What Youll Do

Provide brief, evidence-based interventions (often 68 sessions) focused on improving functioning and supporting behavioral change.Conduct behavioral health assessments and collaborate with primary care teams on shared treatment plans.Offer immediate support during medical visits through warm hand-offs and on-demand consultations.Address a range of issues, including depression, anxiety, trauma, chronic disease management, stress, and substance use.Make referrals to specialty mental health services when higher levels of care are needed.Maintain timely, clear documentation and contribute to a strong culture of team-based care.
Who You Are

You are a California Licensed Clinical Social Worker (LCSW) in good standing.You have experience providing brief, solution-focused therapy in high-volume or primary care settings.You understand the value of trauma-informed, culturally responsive care, and can build trust quickly.You're comfortable thinking on your feet and making clinical decisions in real time.You're not just a clinician - you're a collaborator, educator, and advocate for health equity

Preferred Qualifications

Bilingual in Spanish, Punjabi, or Portuguese (a major plus in our diverse community).Prior experience working in FQHCs, school-based health, or integrated care settings.Familiarity with EHR systems and clinical documentation workflows.
